Who we support
The Resilience Hub supports health, care, and emergency service staff in the Humber and North Yorkshire region. To find out more if the Hub is right for you click here.
You can get in touch with the Resilience Hub team by completing our self-referral. Your referral is completely confidential and your results will only be shared within the Resilience Hub Team. You can find out more about how we keep you and your information safe in the below section on Confidentiality. Your results will help us to decide the level and type of support you may need. This might include simple signposting to tools and resources, support to develop a wellbeing plan, or access to wider advice, support and therapy.
We can only accept referrals from those who work within the Humber and North Yorkshire area. If you are unsure if you are eligible for our service, please contact us via hny.resiliencehub@nhs.net before self-referring. We apologise for any inconvenience.

What happens after I complete my referral?
Once you’ve completed the referral form you will receive an email acknowledgement. You will then be able to speak to one of our team in more detail about how things are and how you’re feeling.
Depending on your responses you’ll either be given access to advice and therapy to meet your needs or receive signposting to appropriate tools and resources to support your wellbeing. We can also help you to develop your own wellbeing plan to support you now and into the future.
On completion of your self-referral form, we may offer to refer you to another service which may be able to offer more specific help. We will always discuss this with you and ask your permission before doing so.
What if I have a problem accessing or completing the questions?
If you encounter a problem, would like the questions in a different language, or would prefer to complete the screening over the phone with a member of the team – please get in touch to access this support.
Confidentiality
Any information you provide remains confidential, but should you indicate risk towards yourself or others, particularly a child or vulnerable adult, or that there is illegal activity taking place, we may need to explore the circumstances further with you. The wellbeing questionnaire does not ask for your employer’s details.
If from the results of your self-referral you are identified as requiring urgent help and we are unable to contact you directly to discuss this within a safe timeframe we may contact your GP, in confidence, in order to ensure alternative support is available to you.
By submitting your email address as part of the questionnaire, you are giving consent for us to contact you in relation to your care. This information will not be shared any further or with other agencies. You can choose whether to consent for us to contact you by phone/SMS in the form.
Please Note: We also use anonymised data as a means of constantly evaluating our service, in order to improve and to plan for any future needs.
How long is the self-referral?
It takes an average of 10 minutes to complete the referral form, but you may wish to allow yourself more time if you find it difficult to think about your emotions at the moment. If you’d like help to complete the questionnaire please get in touch with us.
Completing the self-referral allows our expert team to provide better advice and support, for your emotional wellbeing and mental health.
